Complaint Policy

Effective September 1, 2008

To file a complaint in regard to content, comments, reviews or opinions posted on our web site you must notify by emailing the following information to us at

1. Your first and last name, zip code, and telephone number.
2. A description of the content for which you are filing the complaint, including a link to the page if possible. Also include provider name and date the content was posted if available.
3. Give a brief description of why you are filing the complaint.
4. Your signature, using your full name (computerized signature is acceptable).

Once we receive all the pertinent information, we will review your case and respond. We cannot guarantee that we can resolve your complaint but we will make a good faith effort to help you.
